Prescriptions, Sickness Certificates, and Test Results
Repeat Prescriptions
If your doctor has authorised you to receive repeat prescriptions, you may apply online, by post, fax, or email, or bring your prescription side slip to reception.
Important points to note
- All requests for a repeat prescription will be seen and authorised by a doctor
- The surgery requires 2 WORKING DAYS to process your request. Requests received before 10:30am on a Monday, for example, will be ready for collection after 10:30am on Wednesday.
- The above arrangements also apply to online requests.
Collecting a Prescription
- You may collect your prescription from the surgery yourself, or you may authorise someone to collect it for you.
- Some pharmacies offer a collection and delivery service. If you decide on this option, speak to your chosen pharmacy and
- Formally set up an arrangement
- Advise the pharmacy when you submit your prescription, and when it will be ready for collection.
Please note. Reception will need to be satisfied that you have made an arrangement with the pharmacy before handing over the prescription.

Sickness Certificates
You do not require a doctors sick note for any illness lasting seven days or less. Your employer may, however, require you to complete a self-certification form (SC1) which is available from your employer.
For any illness lasting longer than seven days you will need to see your doctor for them to issue a sick note (F.med3). Self certification forms, for the self-employed, are available at the surgery.
Please note: we do not accept requests for sick certificates by phone. You may request a sick certificate online, or come into the surgery and complete a form at reception, or fax your request to us (see Contact Us for our fax number).
If you wish to enquire about a sick certificate you have already requested, please telephone the surgery between 11am and 12 midday, Monday to Friday.
Only requests from patients entitled to receive a medical certificate from this surgery will be accepted.
When will my certificate be ready
Your certificate will not be available until, at the earliest, the first day of sickness it is expected to cover.
Blood Tests, X-Rays and other Results
How to get your test results
On average please allow 7 working days for your test results to come back from the hospital unless your doctor has advised otherwise.
Please note that x-ray results take longer.
For your test results please telephone the surgery on 01256 479 747. Sometimes we cannot give full results over the phone. If the result is complicated, or if the doctor wants to see you about the result, we will offer you an appointment.
We will only give results to the person who has had the test.
